Initial Permission Form - PRINT, SIGN AND RETURN 

PAYMENTS HAVE TO BE MADE ON TIME AND EACH MONTH SO THAT THE TRAVEL AGENCY CAN PAY THE VENDORS ON TIME. THE ROOMING LIST WILL BE SENT IN BY February 18. IT CAUSES MANY PROBLEMS FOR MS. PETERSEN AND THE TRAVEL AGENCY WHEN YOU ARE NOT ON TIME. PRICES ARE BASED ON THE NUMBER OF STUDENTS (4 = no extra charge) PER ROOM AND THE NUMBER OF PAID PARTICIPANTS ON THE BUS. 

• 26-30 paid participants per bus = $1066

• 31-35 paid participants per bus = $961

• 36-40 paid participants per bus = $885

• 41-45 paid participants per bus = $827

• 46-50 paid participants per bus = $782

Package surcharges:

Triple occupancy in room = $35 additional per person per package

Double occupancy in room = $105 additional per person per package

Single occupancy in room = $315 additional per person per package
